Overview
The post holder is responsible for ensuring the delivery of safe and effective nursing care for the whole practice population.
Clinically, the focus of the role is the delivery of evidence-based practice for patients with acute and chronic conditions, and management and preventative nursing interventions to meet all patients.
You will work collaboratively with the whole general practice team to meet the needs of the patients, you will assist the senior practice nurse in leading and managing all the nursing resources, working closely with the GPs and practice manager to deliver the practice priorities.

Clinical Role

- Make professionally autonomous decisions for which he/she is accountable and provide safe, evidence based, cost effective, individualised patient care

- Manages minor illness and triage using a range of clinical assessment skills including: history-taking, physical examination, cognitive assessment and an approach that fosters a bio-psychosocial model: critical thinking and clinical decision making: making objective and appropriate referrals

- Monitors, Interprets and evaluates patient test results in agreeance with the patient, ensure appropriate course of treatment is initiated, or refer on if required

- Delivers care and support planning and anticipatory care

- Delivers Palliative Care

- Make appropriate referrals to primary, community and secondary health care teams

- Delivers person centred care approaches including supporting self management and behaviour change, practising motivational interviewing, delivering compassionate care and adopting personal outcomes asset- and strength based approaches and co-production

- Role-models the values, behaviours and interactions expected , and ensures patient, family and carer feedback support these in practice

- Understands public health, including health inequalities, epidemiology and prevention

- Utilises a range of skills elated to e-health and technology to enable care and support self-management

- Understands mental health and wellbeing

Essential

- 1st level nursing registration with NMC

- Educated to degree level or equivalent

- Non Medical Independent Prescriber

- V300 non-medical prescriber

- Clinical skills module for assessing diagnosing and treatment for unscheduled care

- Evidence of continuing professional development in relation to chronic disease management including (but not fully inclusive): hypertension, rheumatology monitoring, asthma, COPD, CHD, CVD, LVSD, DIABETES, care of patients with splenectomy

- Child protection education
