Social Worker - Adult Triage TeamGateshead, UK£19 - £24 per hour

About the Role:

Randstad is recruiting for a Social Worker to join the Adult Triage Team in Gateshead. This role involves working on the front line of adult social care, where you will assess and prioritize the needs of adults requiring support, ensuring timely and appropriate intervention. As a member of the triage team, you will play a key role in safeguarding and promoting the welfare of adults by responding effectively to referrals and coordinating initial assessments.

Key Responsibilities:

  • Conduct initial triage and assessments for adults referred to the team, determining priority and urgency based on their needs.
  • Work closely with multi-agency professionals, including health services, housing, and community organizations, to ensure a comprehensive approach to care and support.
  • Provide safeguarding assessments and identify any potential risks, taking appropriate action to ensure the safety of vulnerable adults.
  • Develop and communicate clear, outcome-focused action plans, making referrals to appropriate services as required.
  • Maintain accurate and up-to-date case notes and documentation, ensuring compliance with all relevant legislation and safeguarding procedures.
  • Engage with adults and their families, providing guidance and support to help them understand the assessment process and the resources available to them.

Requirements:

  • Qualified Social Worker with valid Social Work England registration.
  • Previous experience in adult social care, particularly in triage, assessment, or safeguarding roles.
  • Strong understanding of the Care Act, Mental Capacity Act, and safeguarding legislation.
  • Excellent communication and assessment skills, with the ability to make quick, informed decisions.
  • Commitment to delivering person-centered services that respect the rights, independence, and dignity of adults in need.
