We are seeking a highly motivated and experienced Field Care Supervisor to join our team in Gerrards Cross, UK. Responsibilities:
- Conduct regular visits to clients' homes to assess their care needs and ensure the quality of care provided meets our standards
- Supervise and support a team of care workers, providing guidance and training as needed
- Coordinate and schedule care visits for clients, ensuring all shifts are covered and any changes are communicated effectively
- Conduct care reviews and risk assessments, updating care plans as needed
- Act as a point of contact for clients and their families, addressing any concerns or issues promptly
- Monitor and maintain accurate records of care provided and any changes in clients' conditions
- Liaise with healthcare professionals and external agencies to ensure coordinated care for clients
- Participate in on-call duties as scheduled, providing support and guidance to care workers in emergency situations
Requirements:
- Minimum of 1 year experience in a supervisory role in the care sector
- NVQ Level 3 in Health and Social Care or equivalent qualification
- Strong leadership skills and ability to motivate and support a team
- Excellent communication and interpersonal skills
- Knowledge of CQC regulations and standards
- Ability to work independently and manage time effectively
- Valid UK driver's license and access to a vehicle
- Flexibility to work occasional evenings and weekends as needed
