- This role is working for a well-established charity in their Gloucestershire Rough Sleeper Service. The service provides specialist outreach support to adults with a history of rough sleeping across Gloucestershire. It focuses on both the rural districts of Tewkesbury, Forest of Dean, Stroud, and the Cotswolds, as well as the urban areas of Gloucester and Cheltenham.
- Working in partnership with local housing and support organisations, you will be offering holistic services designed to empower clients, helping them build skills, resilience, and independence. This support serves as a stepping stone toward independent living and a move away from street-based lifestyles.
- Identify and build relationships with people who are rough sleeping in Gloucestershire, using outreach sessions and regular visits to agencies/venues visited by people who are rough sleeping.
- Carry out Needs & Risk Assessments with identified rough sleepers to secure access to services which are suitable for their immediate needs.
- Undertake rostered outreach sessions, including early morning, late evenings, and weekends, taking the service to the clients.
- Identify barriers which make it difficult for individuals to access services and find solutions to overcome them.
- Make appropriate referrals in partnership with rough sleepers to establish a sustainable life away from the streets as quickly as possible.
- Become a positive presence and resource in locations such as homeless day centres and drop-ins.
- Complete new and review, needs assessment/risk assessment for clients leading to the delivery of full support plans which meet individual client need.
- Respond to reports of cases of rough sleeping across the county and update within set timescales.
- Give clients advice and information; provide an enabling service so that they can access appropriate services and accommodation.
- Liaise with relevant agencies and, with the agreement of the Client, act as an advocate with those agencies on the Client’s behalf.
- Offer support and advice around reconnections and where relevant, make single service offers.
- Support the delivery of the Severe Weather Emergency Protocol in accordance with local policy
- Participate in street counts/audits as directed by the Service Manager
- Maintain client records that are accurate and fully updated.
- Enhanced DBS issued within the last 12 months or on the update service.
- Empathy & understanding of the needs of individuals who have been rough sleeping.
- Experience working as part of a team in a social or health care setting.
- Understanding of the causes of homelessness and social exclusion and approaches which seek to address this and prevent people from sleeping rough
- Full driver license and access to a car is desirable.
