Responsibilities:
- Conducting assessments of individuals' needs for social care and support, considering physical, emotional, and environmental factors.
- Developing and implementing care plans tailored to everyone's circumstances, promoting independence and well-being.
- Providing advice, guidance, and practical support to individuals and their families/carers regarding available services and resources.
- Liaising with healthcare professionals, community agencies, and other stakeholders to coordinate and deliver integrated care services.
- Safeguarding vulnerable adults by identifying and responding to instances of abuse, neglect, or exploitation, following established protocols and procedures.
- Advocating for clients' rights and interests within the social care system, ensuring they receive appropriate support and access to necessary resources.
- Maintaining accurate and up-to-date records of assessments, interventions, and outcomes in accordance with regulatory standards and organisational policies.
- Participating in multidisciplinary team meetings, case conferences, and training sessions to enhance professional development and service delivery.
Qualifications and Skills:
- A degree in Social Work or equivalent qualification recognised by Social Work England.
- Registration with the SWE as a qualified Social Worker.
- Knowledge of relevant legislation, policies, and procedures governing adult social care in the UK.
- Strong communication, interpersonal, and problem-solving skills.
- Ability to work independently and collaboratively within a diverse and demanding environment.
- Empathy, resilience, and a commitment to promoting social justice and equality.
- Proficiency in IT systems used for case management and record-keeping.
