- Supporting a caseload of 15 children and young people.
- Work with potential referrers to identify appropriate referrals to the service.
- Implement effective assessment and goal setting with identified children and young people.
- Develop a programme of support for each child or young person, to include delivery of the evidence-based programmes for resilience, healthy relationships etc (as appropriate).
- Develop a good working relationship with schools, children, families, and other agencies to ensure that the service is effective.
- Provide relevant support for school-based staff and staff in other universal settings to ensure identified children and young people are supported across these settings.
- Work directly with parents to ensure that they are better able to understand/ support their child’s needs and support the development of resilience and improved well-being.
- Explore all opportunities for talents and interests to be developed using local resources.
- Ensure a good level of support to children, young people and families, including onward referral to other agencies where necessary.
- Keep clear accurate records and provide the required monitoring and evaluation information and any other progress reports required by due dates.
- Implement evaluation tools to demonstrate impact of intervention.
- Attend monthly supervision with for quality assurance and personal development purposes.
- Consider own training needs, keeping up to date with relevant good practice, and sharing this with team members.
