The role:

  • To work as a member of the Prevention and Support Service to provide support to children, young people and their families.
  • To work as a Lead Professional as part of a 'team around the family'.
  • To support the development, delivery and co-ordination of an agreed plan

KEY SERVICE RELATED ACCOUNTABILITIES

  • To provide practical assistance, support and guidance to parents to enable them to care and protect their children successfully.
  • To support families to establish good local community networks and support mechanisms to promote their self-help skills and resilience
  • To encourage parents to keep medical and other appointments linked to their families' development, which may include transporting them to appointments and meetings.
  • To liaise as appropriate with other professionals and key people, e.g. Foster Carer, Health Visitors, Schools, Housing and Education Departments

The Candidate:

  • To be able to quickly establish effective working relationships with service users, partner agencies and other professionals.
  • To be able to work directly with children and young people using a variety of methods and interventions.
  • Ability to plan and organise workload
  • To work with own initiative and as a team member
  • Ability to communicate and liaise with colleagues from other agencies in the statutory, community/voluntary sector.
