Position: Community Psychiatric Nurse (CPN)Location: Surrey Heath Community Mental Health Team (CMHT)Pay Rate: £28 per hour (paid via umbrella)Contract Type: Agency, TemporaryHours: Full-time, 37.5 hours per week Role Overview:We are seeking experienced and motivated Community Psychiatric Nurses (CPN) to join the Surrey Heath Community Mental Health Team (CMHT) on a temporary agency basis. As a CPN, you will provide high-quality mental health care and support to individuals within the community who are experiencing mental health issues. You will be responsible for delivering a range of clinical and therapeutic interventions while working closely with multidisciplinary teams to support service users.Key Responsibilities:
  • Assessment and Care Planning:Conduct comprehensive mental health assessments and create personalized care plans tailored to the needs of individuals with a variety of mental health conditions.
  • Clinical Interventions:Provide direct nursing care, therapeutic interventions, and psychosocial support to service users within the community setting.
  • Collaboration:Work closely with multidisciplinary teams, including psychiatrists, social workers, and occupational therapists, to ensure coordinated and holistic care.
  • Monitoring and Evaluation:Regularly review and monitor the progress of care plans and interventions, making adjustments as required based on the individual's needs.
  • Crisis Management:Respond to mental health crises, including risk assessments and the management of high-risk situations.
  • Documentation:Maintain accurate and up-to-date records of all assessments, interventions, and communications in accordance with policies and professional standards.
  • Safeguarding:Actively identify safeguarding concerns and ensure that appropriate actions are taken in line with safeguarding procedures.
Essential Qualifications and Requirements:
  • Registered Nurse (Mental Health) (RMN):Must be a registered Mental Health Nurse with the NMC (Nursing and Midwifery Council).
  • Experience:Minimum of 1 year experience working as a Community Psychiatric Nurse (CPN) within a Community Mental Health Team or similar mental health setting.
  • Skills:Strong clinical assessment and risk management skills. Ability to work independently and manage a caseload effectively.
  • Knowledge:In-depth understanding of mental health disorders, care planning, and relevant legislation, including the Care Act and the Mental Health Act.
  • DBS Check:A current enhanced DBS (Disclosure and Barring Service) check will be required before starting the role.
Desirable:
  • Experience working with adults in a community setting with diverse mental health needs.
  • Additional mental health or clinical qualifications.
