As a Care Coordinator, you will play a vital role in ensuring the smooth operation of care services.

Your responsibilities:

  • Coordinating and Scheduling Care:Ensure clients receive the right care at the right time.
  • Liaising with Stakeholders: Work closely with clients, their families, and care staff to assess needs and develop customised care plans.
  • Monitoring Care Delivery: Ensure that care meets quality standards and regulatory requirements.
  • Maintaining Accurate Records: Keep detailed records of client care plans, progress, and any changes in requirements.
  • Supporting Care Staff: Provide guidance and resources to ensure exceptional care delivery.
  • Collaborating with Professionals: Work with health-care providers and community organisations to enhance the overall care experience.

Skills/ Experience:

  • Previous experience in care coordinationor a related role.
  • Excellent organisational skills with the ability to prioritise tasks and meet deadlines.
  • Strong communication and interpersonal skills to effectively engage with clients, families, and care staff.
  • Knowledge of care regulations and industry standards.
  • Problem-solving abilities and adaptability in changing situations.
  • Proficiency in using computer systems and software for record-keeping and scheduling.
