Daytime healthcare are working with an established Home Care agency, who are expanding and looking for an experience Care Coordinator to join their friendly team!

You could have the opportunity to work alongside a National Care provider!

Duties include;

  • Responsible for carrying out initial assessments, doing reviews and spot checks
  • Creating service user care plans. Planning staff Rota``s using the bespoke system
  • Supporting the new and existing care staff team and coordinating the key carers in your designated areas
  • Ensuring that all operations are Company and CQC Compliant

Requirements;

  • You must have a full valid Driving Licence with access to a vehicle
  • Good organisational and communication skillls to ensure an excellent service
  • Experience doing rotas and care plans
