Role Overview:
We are seeking a highly organized and experienced Care Coordinator to join our team. In this role, you will be responsible for the day-to-day coordination of care services, ensuring that our clients receive timely, efficient, and compassionate care tailored to their individual needs. You will act as the primary liaison between clients, caregivers, and families, ensuring effective communication and a high standard of care delivery.

Key Responsibilities:

  • Schedule and coordinate care visits, ensuring optimal allocation of carers to clients based on skills, availability, and client needs.
  • Maintain accurate and up-to-date care plans, ensuring any changes in client needs are promptly addressed.
  • Act as the first point of contact for both clients and care staff, managing queries and resolving issues.
  • Monitor the quality of care being delivered through regular client feedback and assessments. Client Retention & Relationship Management: Build and maintain strong relationships with clients and their families, addressing their concerns and ensuring client satisfaction and retention.
  • Support recruitment, induction, and ongoing training of care staff.
  • Ensure all care services comply with CQC regulations.
  • Collaborate with other departments and health professionals to ensure holistic care for clients.

Qualifications and Skills:

  • Previous experience in a similar role within the health or social care sector, preferably in domiciliary care.
  • Excellent organizational and time-management skills, with the ability to manage multiple schedules.
  • Strong communication skills, both verbal and written, with a professional and compassionate approach.
  • Knowledge of CQC regulations and care standards.
  • IT proficient, with experience in scheduling software or systems being an advantage.
  • A caring and empathetic nature with a passion for supporting vulnerable individuals.
  • Ability to work flexibly, including on-call duties when necessary.
