Connect2socialwork are urgently looking for a Social Worker to join their great Hospital Discharge with a Pay rate up to 35 an hour on a locum basis.

Job Description;
Work collaboratively to ensure safe and efficient hospital discharges.
Manage a complex caseload, working independently and efficiently.
Provide support and intervention for individuals and families during transition from hospital to community-based care.
Liaise with multidisciplinary teams to ensure seamless and person-centered care.
- SWE Registration
- Eligible to work in the UK
- Drive and have access to a car
- Post-qualified experience
- Degree level or equivalent in Social Work
