This is a fantastic opportunity for a passionate and experience Practice Nurse to join a highly reputable GP Training Practice for 3-4 days per week. Benefit from working alongside a multidisciplinary team renowned for providing first class service to patients. The practice is very high performing with CQC and QOF and this is a fantastic opportunity for someone experienced with routine Practice Nurse duties, as well as experience with Chronic Disease Management.
Salary – £35,000 - £38,900 per annum DOE FTE (£17.95-£19.95 per hour) + NHS pension + 27 days annual leave + Bank Holidays + Employee Assistance Program

Your role –
- Essential experience as a Practice Nurse in Primary Care comfortable with all routine Practice Nurse responsibilities and at least one or two diplomas in Chronic Disease Management.
- Support with career development.
- Need to be experienced with SystmOne.
- Benefit from a very supportive MDT around you.
- 3-4 days per week.
- 20 minute paid coffee breaks during the morning and afternoon sessions.
