Position: Care Coordinator

Salary: 28000 per annum
Type: Permanent, full-time
We are currently seeking a highly motivated and compassionate Care Coordinator to join our domiciliary care provider located in Harrogate. As a Care Coordinator, you will be responsible for coordinating and managing the delivery of care services to our clients in the community. The ideal candidate will have excellent organizational and communication skills, as well as a passion for providing high-quality care to individuals in need.
Key Responsibilities:
Coordinate and manage the delivery of care services to clients in the communityConduct initial assessments and create personalized care plans for each clientLiaise with clients, their families, and other healthcare professionals to ensure the best possible care is providedSchedule and assign care staff to clients based on their needs and preferencesMonitor and review care plans regularly to ensure they are meeting the clients' needsMaintain accurate and up-to-date records of clients' care and progressProvide support and guidance to care staff to ensure they are delivering care in line with company policies and proceduresParticipate in on-call duties and provide support to care staff as neededAssist with recruitment and training of new care staffCollaborate with the management team to continuously improve the quality of care services providedRequirements:
Previous experience in a similar role within the healthcare sectorNVQ Level 3 in Health and Social Care or equivalentStrong organizational and time management skillsExcellent communication and interpersonal skillsAbility to work independently and as part of a teamKnowledge of CQC regulations and standardsProficient in Microsoft Office and other relevant computer systemsValid UK driver's license and access to a vehicleWe Offer:
