EHC Transition CoordinatorLocation: Harrow Weald Job Type: Full-timeSalary: £39,805 to £41,892 per annum including London WeightingContact: 1 permanent, 1 fix term till Aug 2025Day-to-day of the role:

  • Coordinate the EHC needs assessment process, including consultations and annual reviews for individual learners.
  • Develop and maintain partnerships with parents, practitioners, and young people to ensure effective service delivery.
  • Act as the main point of contact for parents, young people, and SEND Caseworkers, organising and attending meetings related to EHC planning and transitions.
  • Ensure that Individual Support Plans accurately reflect the EHCP recommendations.
  • Monitor the progress of learners towards expected outcomes through ongoing liaison with teaching and support staff.

Required Skills & Qualifications:

  • Proven experience in coordinating EHC plans and working within the SEND framework.
  • Strong organisational and communication skills, with the ability to manage multiple tasks efficiently.
  • Experience in building and maintaining effective partnerships with various stakeholders.
  • Ability to monitor and report on the progress of individual learning plans.
  • Knowledge of current legislation and best practices in special educational needs and disability support.
