Hartlepool Borough Council is seeking an experienced agency social worker to join our Disabilities Team. This is a fantastic opportunity for someone with a passion for supporting individuals with disabilities to make a positive difference in their lives.
Key Responsibilities:
- Provide high-quality social work support to individuals with disabilities.
- Assess the needs of service users and create tailored care plans that promote independence and well-being.
- Work collaboratively with families, carers, and multi-disciplinary teams to ensure the best outcomes for service users.
- Manage a caseload of complex cases while maintaining high standards of practice.
- Ensure compliance with relevant legislation and policies, providing safeguarding support where necessary.
Requirements:
- Experience: A minimum of 2 years' experience as a social worker, ideally within a disabilities setting.
- Qualifications: A recognised social work qualification and HCPC registration.
- Knowledge: Understanding of relevant legislation and frameworks, including the Care Act and safeguarding protocols.
- Hybrid Working: Hartlepool Borough Council has adopted a hybrid working model. Flexible working arrangements will be discussed with the manager.
- Communication Skills: Excellent interpersonal and communication skills to engage with service users, families, and multi-agency teams.
