Your Role at a Glance

As a Health Services Coordinator, you'll play a key role in ensuring exceptional care delivery. You'll collaborate with healthcare professionals, guide patients and families, and be the driving force behind personalised care plans that enhance lives.

What You'll Be Doing

  • Designing and adapting personalised care plans alongside healthcare professionals
  • Monitoring and assessing care quality to maintain top standards
  • Organising training for new staff and development opportunities for existing team members
  • Providing patients and families with education and guidance on available services
  • Partnering with community resources to expand support networks
  • Promoting health and wellness initiatives within the community
  • Gathering and analysing patient feedback to drive continuous improvement
  • Responding swiftly to urgent care concerns, ensuring patient safety at all times
  • Keeping meticulous records and documentation of care services and activities

What We're Looking For

  • Essential: A full UK driving licence and access to your own vehicle
  • Previous experience in a similar healthcare role
  • Level 3 Health and Social Care qualification (preferred)
  • Familiarity with care sector systems
  • Outstanding communication and interpersonal skills
  • Strong organisational abilities and a proactive mindset
  • Composure and effectiveness under pressure
  • A team player with the confidence to work independently
