What You'll Be Doing:

Join our team as a Community Mental Health Practitioner and play a key role in supporting adults in the community with complex mental health needs. In this role, you will:

• Perform comprehensive patient assessments• Create and implement personalised care plans• Deliver evidence-based treatments to optimise patient outcomes• Collaborate with multidisciplinary teams to provide holistic care• Maintain accurate patient records

Requirements & Qualifications:

To be successful in this role, you will need:

• A current and valid NMC or Social Work England registration• Experience working with adults in community mental health settings• Strong communication and interpersonal skills• The ability to work independently and within a multidisciplinary team• A commitment to delivering high-quality patient care

Working Hours & Contract Duration:

This full-time role runs Monday to Friday, 09:00-17:00, for an initial 3-month contract, with a strong likelihood of extension based on performance and service needs.
