Band 6 Community Psychiatric Nurse (CPN) - Hereford based - NHS

Location: Hereford
Working Hours: Monday to Friday, 9:00 AM - 5:00 PM
Contract Length: Minimum of 4 months, with potential extension to 4-6 months ongoing
Start Date: December 2024 - January 2025
Essential Requirement: Must be a car driver

What You’ll Do:

• Provide high-quality, patient-focused mental health care to individuals within the community.
• Conduct thorough assessments, develop tailored care plans, and deliver effective interventions.
• Work collaboratively with multidisciplinary teams to ensure holistic and integrated care.
• Maintain a manageable caseload while delivering timely and efficient service.
• Keep accurate, professional documentation in line with NHS standards.

What We’re Looking For:

• Essential: Proven experience in a community mental health setting—this is a must!
• Current NMC registration as a Mental Health Nurse (Band 6).
• A valid UK driving license and access to a vehicle.
• Exceptional communication, organizational, and problem-solving skills.
• A motivated self-starter who thrives in dynamic environments.
