• Manage an allocated caseload, ensuring that all service users receive high-quality and personalised care.
• Complete referral forms and liaise with teams for smooth client transitions between services.
• Maintain accurate records and report any incidents in adherence with Trust Policy.
• Safeguard service users by following established procedures and raising alerts when necessary.
• Administer medication safely and according to legal and Trust guidelines.
• Provide leadership and supervision to junior staff, enhancing their skills and practices.
• Coordinate with multidisciplinary teams and external agencies to deliver comprehensive care.
• Engage in regular practice reflection and supervision, fostering a culture of continuous improvement.
