Position: Community Occupational Therapist – Regaining Independence ServiceDirectorate: Reablement ServicesLocation: Horsham (Hybrid - home working and community visits)Job OverviewPertemps are recruiting for a qualified and experienced Occupational Therapist to join a dynamic and supportive Occupational Therapy team within the Regaining Independence Service in Horsham. This role is dedicated to enhancing clients' independence through strengths-based assessments and tailored support plans aimed at improving well-being and achieving client-centred outcomes.Key Responsibilities:
  • Case Management: Manage a caseload of clients, collaborating with individuals, families, and carers to create personalized, outcome-focused support plans.
  • Strengths-Based Assessments: Conduct strengths-based assessments and reviews, applying relevant legislation to identify eligible needs and define personalized goals.
  • Support Planning: Develop and implement support plans using Occupational Therapy expertise, community resources, and assets to meet clients’ unique needs and promote their well-being.
  • Community Engagement: Collaborate effectively with internal team members, voluntary agencies, and other local services to provide seamless support.
Qualifications and Skills:
  • Occupational Therapy Experience: Proven background in strengths-based assessments, support planning, and reablement services.
  • Communication & Partnership: Ability to work effectively with clients, families, and partner agencies, applying a strengths-based approach.
  • Driving: A valid driving license and access to a car for home and community visits.
  • Professional Standards: Commitment to upholding HCPC and Royal College of Occupational Therapists professional standards.
