What you will do:
- Provide Social Work and Care Management services to residents and carers in Hounslow.
- Conduct Social Care Assessments to identify needs and desired outcomes, ensuring that residents have a voice in the process.
- Promote and protect the rights and interests of residents and their carers, maintaining their safety in accordance with the Council’s and Pan London Safeguarding Policy.
- Participate in investigations and contribute to strategy meetings and action planning.
