About the Role:

We are seeking an experienced Advanced Clinical Practitioner to join an Urgent Community Response Team in Kirklees. This role is pivotal in providing high-quality care to patients in their own homes and care homes, ensuring timely and effective clinical interventions.

Key Responsibilities:

  • Conduct initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.
  • Demonstrate safe, clinical decision-making and expert care, including advanced assessment and diagnostic skills.
  • Provide care for patients in their own homes and care homes.
  • Support in an acute environment when required to facilitate discharge.

Requirements:

  • Proven experience as an Advanced Clinical Practitioner.
