Job Advert: Long-Term Condition Review Clinician - Kirklees AreaLocation: Kirklees, HuddersfieldSalary: £28 to £30 per hour Paye or LtdContract Type: Ongoing upto 5 days per week until March 2025 with view to extensionWe are currently seeking experienced clinicians to join our Long-Term Condition Review Service. In this role, you will be carrying out face-to-face domiciliary reviews for patients who are unable to attend their GP practice, visiting patients in their homes within the Kirklees area.Key Responsibilities:

  • Conducting long-term condition reviews (e.g., blood pressure, observations, and blood results).
  • Assessing clinical conditions and providing recommendations for management.
  • Conducting medication reviews, including evaluating side effects, efficacy, suitability, and continued need.
  • Taking relevant observations such as blood pressure and diabetic foot checks (training provided if required).
  • Making recommendations for medication changes (prescribing is not required).

Key Requirements:

  • Experience in delivering long-term condition reviews, ideally within primary care. Experience in practice environments will also be considered.
  • Previous experience or qualifications in respiratory conditions (COPD, asthma), diabetes, and/or cardiac conditions (heart failure, hypertension, coronary heart disease) is an advantage.
  • Confidence in making medication recommendations and managing long-term conditions.
  • Ability to perform basic clinical assessments and take relevant observations.
  • Additional qualifications or diplomas in relevant fields are a plus.

Ideal Candidates:We are particularly interested in candidates with primary care nursing experience or specialist pharmacists with experience in managing long-term conditions.
