Shift Availability: 7 days a week, tailored to your availability

Role Overview: We are seeking a dedicated and experienced pharmacist to join our team on a temporary basis. In this role, you will conduct long-term condition reviews for patients, primarily focusing on COPD, asthma, diabetes, heart disease, heart failure, and hypertension. While proficiency in all these conditions is not required, experience in them is beneficial.

Key Responsibilities:

  • Conduct comprehensive reviews for patients with long-term conditions.
  • Travel to patients’ homes for assessments.
  • Utilize provided clinical and IT equipment to complete assessments.
  • Maintain accurate and detailed patient records.

Requirements:

  • Valid HCPC registration 
  • Experience in managing long-term conditions (COPD, asthma, diabetes, heart disease, heart failure, hypertension) is preferred.
  • Ability to work independently and manage your schedule.
  • Excellent communication and interpersonal skills.
