Job Title: Assistant Practitioner (Band 4) - November start!

Location: Kirklees, UK

Hours: 37.5 hours per week

Reports To: Clinical Lead

Job Purpose:

We are seeking a dedicated Assistant Practitioner to join our client's multi-disciplinary community and acute-based service. This role involves working with adults with complex physical and neurological disabilities, providing crisis intervention to reduce hospital readmissions and facilitate early discharge from A&E and Frailty settings.

Key Responsibilities:

  • Contribute to intermediate care and unplanned services.
  • Work independently with clients on treatment programmes developed under the supervision of qualified therapists and nurses.
  • Assist in the assessment, treatment, and evaluation of clients’ needs to avoid unnecessary hospital admissions and ensure swift discharge.
  • Implement treatment programmes and provide necessary equipment.
  • Conduct initial assessments of patients referred following a fall and report to senior clinicians.
  • Maintain communication with therapists and nurses, reporting any changes in patients’ conditions.
  • Achieve competencies in initial assessments and work with Occupational Therapy, Nursing, and Physiotherapy to deliver individual treatment programmes.
  • Support the smooth running of the ITOC through a flexible and adaptable approach to work, including administrative tasks.

Clinical Responsibilities:

  • Achieve advanced competencies for falls assessments and multi-disciplinary working.
  • Assist in the day-to-day delivery of the ITOC team services, including hospital discharge, A&E, Frailty, and Discharge to Assess.
  • Participate in team meetings and develop an understanding of the roles within the team.
  • Assist in moving and handling clients using specialist equipment.
  • Support clients and carers by implementing individual therapy programmes and providing information for treatment evaluation.
  • Manage challenging behaviour and report relevant information to qualified staff and agencies.
