Job Description:

We are seeking an experienced and compassionate Band 6 Primary Care Mental Health Nurse (CPN) to join our growing team in the East Midlands. This role offers an exciting opportunity to work autonomously across designated GP practices, providing high-quality, recovery-focused psychological interventions. You will support service users who present with complex mental health needs that do not meet the criteria for secondary care but are beyond the scope of standard Talking Therapies.

Key Responsibilities:
  • Deliver Specialized Interventions: Provide tailored, recovery-focused psychological interventions to patients with complex mental health needs, bridging the gap between primary and secondary care services.
  • Autonomous Clinic Management: Operate with a high level of autonomy, managing your own clinics within GP practices, ensuring efficient and compassionate patient care.
  • Patient Assessment & Care Planning: Conduct comprehensive mental health assessments, develop personalized care plans, and explore a range of treatment options to meet individual patient needs.
  • Liaison & Collaboration: Work closely with GPs, mental health teams, and a wide range of statutory and non-statutory organizations to ensure a holistic approach to patient care.
  • Risk & Therapeutic Management: Perform detailed risk assessments and deliver therapeutic interventions in an accessible manner for patients who may struggle with more formal, structured approaches.
  • Caseload & Referral Management: Effectively manage your own caseload, triage non-urgent mental health referrals, and signpost or refer clients to appropriate services as needed.
  • Team Contribution: Play an active role in the development and continuous improvement of our team, contributing ideas and strategies to enhance patient outcomes.
