Key Responsibilities:
- Provide comprehensive pharmacy services in both inpatient wards and outpatient settings.
- Conduct medicines reconciliation, clinical screening, and oversee the dispensing and administration of medications.
- Collaborate with the wider multidisciplinary healthcare team to ensure the delivery of safe and effective patient care.
- Advise on appropriate treatment regimes, patient safety, and adherence to regulatory and hospital guidelines.
- Provide clinical input on complex patient cases and ensure continuity of care during hospital stays.
Requirements:
- Must have at least 2 years of ward and hospital experience as a pharmacist.
- GPhC registration is essential.
- Strong understanding of clinical pharmacy in a hospital setting, including medicines management and patient safety practices.
- Excellent communication skills and ability to work well within a multidisciplinary team.
- Flexibility to adapt to the fast-paced hospital environment.
