As a Social Worker in the North Community Team, you will support adults with a range of complex needs, including physical disabilities, mental health challenges, and learning disabilities. Your responsibilities will include assessing their needs, creating individualised care plans, and facilitating access to services that enhance well-being. This role emphasises delivering person-centred support while ensuring compliance with statutory requirements under the Care Act 2014.

Responsibilities include:

  • Participate in assessments and care planning and safeguarding under the supervision of an experienced practitioner.
  • Carry out mental capacity assessments in accordance with the Mental Capacity Act 2005. Make applications for Deprivation of Liberty Safeguards (DoLS) where appropriate, ensuring legal compliance and protecting individuals' rights
  • Responsible for managing a caseload, conducting robust assessments in partnership with service users, their carers and multi-disciplinary partners where appropriate
  • Maintain accurate, clear, and timely records of assessments, interventions, and care plans, ensuring all documentation is in line with council policies and legal standards

Requirements

  • Relevant social work qualification
  • 2 experience in adult social work, including assessment and care planning.
