As a Social Worker in the Integrated Adult Independence Team, you will be dedicated to working with adults to promote their independence, well-being, and overall quality of life. Your responsibilities will include conducting thorough assessments under the Care Act 2014 and other relevant legislation, ensuring that the needs of vulnerable adults are met. You will focus on enabling adults to live independently, safeguarding their well-being, and providing tailored support to address their individual circumstances.



Responsibilities include:

  • Perform assessments according to the Mental Capacity Act.
  • Carry out mental capacity assessments in accordance with the Mental Capacity Act 2005.
  • Examine and supervise carer strategies.
  • Responsible for managing a caseload, conducting robust assessments in partnership with service users, their carers and multi-disciplinary partners.
  • Work closely with the Team Manager with a strengths based approach to provide personalised services with a clear focus on quality outcomes for our residents.



Requirements:

  • 2 years experience post qualification
  • Action safeguarding procedures
