Job Description: We are currently seeking an experienced and motivated Care Home Support Worker to join our team. As a Care Home Support Worker, you will play a key role assisting service users in a care home setting, working in team of support workers to deliver high-quality care and support to our clients.

Responsibilities:

* Providing support and care to elderly service users in a care home environment
* Coordinating and allocating work tasks, ensuring that support is delivered effectively and in line with individual care plans
* Providing hands-on support to individuals in a care home setting
* Undertaking assessments and reviews of individual care plans
* Liaising with families, carers, and external professionals to ensure a coordinated approach to care
* Monitoring and reporting on the progress and well-being of clients
* Supporting with staff training, development, and supervision

Requirements:

* Previous experience working in a similar role within the social care sector
* NVQ Level 3 in Health and Social Care or equivalent
* Experience of working with individuals with elderly care needs
* Strong leadership and management skills
* Excellent communication and interpersonal skills
* Ability to work effectively as part of a team
* A commitment to delivering high-quality, person-centered care
