Experienced Support Worker required to work as part of the Community Support Team in the Ipswich area. This role requires no PERSONAL CARE or MEDICATION ADMIN but it is essential that you are a CAR DRIVER.

You will be based from one site in the IP1 area and work as part of the wider team supporting services users in their own homes to prevent hospital admissions.

* Assessing service users needs

* Completing and following Support Plans

* Provide practical and emotional support in service users own homes

* Referrals received from Clinicians and Community Health Professionals and you and the team will then be responsible for assessing the service users needs and requirements in order to prevent hospital admission

Profile

* Have Support Worker experience

* CAR DRIVER IS ESSENTIAL and access to own vehicle

* Able to work as part of a team but confident to work in the community

* Basic IT Skills

NO PERSONAL CARE OR MEDICATION ADMINISTRATION IS REQUIRED FOR THIS ROLE

* Client will require that you complete their Mandatory Training

Job Offer

* Monday to Friday 10am - 5.30pm

* 32.5 Hours/Week

* Occasional weekend cover

* £12/Hour

* This position is subject to an Enhanced Adult DBS (can be applied for if your not on the Update Service)

* Starting Monday 7 October for approximately 12 Weeks (may be extended)
