Your new companyOxfordshire County Council are looking to recruit a number of Qualified Social Workers for their Discharge to Access (D2A) Home First Service.

Your new roleOxfordshire County Council hospital discharge services are undergoing an exciting transformation to support the residents of Oxfordshire in having their needs assessed and met in their own homes, rather than at a hospital bedside.This community-based - Home First D2A role, as a new service, we encourage creative thinking, ideas and innovations to support the development of the service from all our employees.As part of our team, you will:

  • Work closely with our reablement providers and the voluntary sector. You will support and guide people to regain independence in their own home using a strength-based approach to goal planning during short-term intermediate care provision.This intervention will be provided following discharge from hospital or to help prevent admission to hospital.
  • Draw upon your skills and knowledge in activity analysis, assistive technology, aids and minor adaptations to support the reduction in formal care wherever possible.Moving and Handling skills will also be required.
  • You will have a good understanding of the discharge to assess process, be competent in MCA and urgent Social Work assessments, with the ability to work within the West and Vale area of Oxfordshire.

This role will pay depending on experience, on an ongoing contract. JD is available upon request.

What you'll need to succeedTo be successful, you will need to have a recognised professional Social Work qualification (MA/BA/B.Sc. in Social Work, Diploma in Social Work or CQSW) and Social Work England registration along with 1 year post qualifying experience.
