About the Role

In this vital role, you will play a key part in supporting individuals as they transition from hospital care to home. Key responsibilities include:

Conducting comprehensive occupational therapy (OT) assessments for individuals discharged to short-term beds from the acute hospital.
Collaborating with multidisciplinary teams to create and implement personalized care plans.
Facilitating timely and safe discharges from short-term beds, ensuring a smooth return home for patients.
Providing expert guidance and recommendations to optimize patient independence and quality of life.

What We’re Looking For:

Qualifications: Degree in Occupational Therapy and current HCPC registration.
