About the Role

In this vital role, you will play a key part in supporting individuals as they transition from hospital care to home. Key responsibilities include:

  • Conducting comprehensive occupational therapy (OT) assessments for individuals discharged to short-term beds from the acute hospital.
  • Collaborating with multidisciplinary teams to create and implement personalized care plans.
  • Facilitating timely and safe discharges from short-term beds, ensuring a smooth return home for patients.
  • Providing expert guidance and recommendations to optimize patient independence and quality of life.
