An exciting new opportunity has arisen for an Experienced Social Worker to support disabled children and their families in Disabled Children's Team. This post will be based in Kings Hill but there is an expectation to work across the whole of the West Kent area.

As an experienced social worker, you will bring your expertise and passion to our Disabled Children's Team. You will play a crucial role in assessing needs, developing and implementing care plans, and delivering effective interventions. This position offers the opportunity to work in a supportive and collaborative environment where your experience and insights will drive meaningful change

Key Responsibilities:

- Conduct in-depth assessments of children with disabilities and their families.

- Develop, implement, and review personalized care plans that address individual needs and circumstances.

- Work collaboratively with multi-disciplinary teams, including healthcare providers, educators, and community organisations.

- Provide direct support, advocacy, and guidance for children and families.

- Maintain detailed and accurate case records in compliance with legal and organisational standards.

- Offer mentorship and support to newly qualified social workers and other team members.

- Engage in regular supervision and ongoing professional development to stay current with best practices and policy changes.
