About the role: As a Social Worker in the Children with Disabilities Team, you'll play a vital role in improving the lives of children and young people with disabilities. Your responsibilities will include:
- Assessing the needs of children and young people with disabilities, determining if they require specialist intervention or can access mainstream activities.
- Evaluating the needs of children with severe and profound disabilities, specifically focusing on short breaks support for them and their families.
- Creating care plans for children in various situations, ensuring their needs are met by all involved in their care.
- Conducting home, school, and community visits, with a focus on direct work with children and young people using suitable communication methods.
- Collaborating with health, education, and other professionals to ensure the comprehensive needs of children and young people are met.
About you:
- Hold a Social Work qualification and have completed your Assessed and Supported Year in Employment (ASYE). ESSENTIAL
- Be registered with Social Work England. ESSENTIAL
- Ideally, possess experience working directly with children and young people with significant disabilities. However, experience in safeguarding is also valuable.
- Be willing to work across two London Boroughs (Richmond and Kingston) while being based in Chessington.
- Be comfortable working in an office environment, with a hybrid working model available.
- Be a team player, actively participating in the team duty system, supporting colleagues and other professionals, and maintaining a positive attitude towards your work.
