We are looking for someone that enjoys being part of an integrated approach in working with discharge planning from Hospital. They should have a good knowledge of Continuing Health Care and the assessment process and working within the Discharge to Assess model.

- Best interest decisions and safeguarding all form part of the work that is supported within the team.
We have a permanent duty role in place so this isn't generally required as part of the post.
The role is hybrid so there can be mix of working from home and office based time as appropriate. It is important that you feel part of a team and are supported as you establish yourself in the role. We will consider any applicants home location and try to limit travel wherever possible.
Requirements:
- Degree in Social Work or equivalent qualification.
- Registration with Social Work England.
- Experience in statutory social work functions, including safeguarding and support.
- Strong interpersonal and communication skills, with the ability to work collaboratively with diverse stakeholders.
- Knowledge of relevant legislation, policies, and procedures related to social work practice.
- Commitment to promoting social change, safeguarding vulnerable individuals, and maximising independence.
