Job Overview:We are seeking an experienced Band 6 Occupational Therapist to join the Early Supported Discharge (ESD) team at Ashford Hospital. The role involves supporting patients in transitioning safely and effectively from acute care to home, with a focus on stroke and neurological rehabilitation.

Key Responsibilities:

  • Carry out comprehensive assessments for patients ready for early discharge

  • Design and deliver personalised rehabilitation plans in the community setting

  • Work closely with the MDT to coordinate smooth discharges and follow-up care

  • Provide patient education, carer training, and ongoing therapy reviews

  • Maintain accurate documentation and contribute to service improvement

Requirements:

  • HCPC registered Occupational Therapist

  • Previous NHS experience in stroke or community rehabilitation (minimum 6 months)

  • Strong clinical reasoning and assessment skills

  • Ability to work independently in patients' homes and across community settings

  • Full UK driving licence and access to a vehicle (desirable)
