Essential Requirements:

  • Experience with neurological conditions such as stroke, head injury, Parkinson's disease, and multiple sclerosis.
  • Background in rehabilitation and long-term condition management.
  • Community experience and ability to work with adults and older adults.
  • Effective teamwork skills and capability to complete community visits in Sutton, South West London.
  • Car driver with a personal vehicle required.

Desirable Attributes:

  • EMIS training and NHS SMART card.
  • Experience with patients from a Sutton GP, including in various settings.

Role Overview:

  • Conduct assessments and provide treatment for patients with neurological conditions in the community.
  • Manage and document community visits efficiently.
  • Mileage expenses reimbursed; parking permits not provided for bank or agency staff.
