If you're passionate about making a difference in adult services and thrive in a multidisciplinary environment, this could be the role for you!The Role:You'll provide specialised support to adults facing mental health challenges. Your work will focus on assessment, care planning, and delivering interventions that promote recovery and social inclusion. You’ll be a key player in a multidisciplinary team, ensuring that every individual receives the best holistic, person-centred care.What You'll Be Doing:
  • Assessment & Care Planning: Conduct detailed assessments, including risk assessments, to develop and review personalised care plans.
  • Direct Support & Intervention: Offer therapeutic support, crisis intervention, and advocacy, ensuring service users access the right resources.
  • Multi-Agency Collaboration: Work with healthcare providers, housing agencies, and community services to deliver cohesive care.
  • Safeguarding & Risk Management: Identify safeguarding concerns and manage mental health risks with established protocols to ensure well-being.
  • Documentation & Record-Keeping: Keep accurate, up-to-date records of all assessments, care plans, and interventions.
