Job OverviewWe are currently seeking a dedicated and skilled Band 6 Occupational Therapist for a community-based role at Leeds Community Healthcare. The successful candidate will work across various community settings, providing patient-centered care and promoting independence.
Key Responsibilities- Assess, plan, and deliver occupational therapy interventions for patients in the community.
- Work with multi-disciplinary teams to support the rehabilitation and recovery of patients.
- Develop and implement tailored care plans to meet individual needs.
- Provide advice and education to patients, families, and carers.
- Manage a caseload, ensuring timely and effective interventions.
- HCPC registration as an Occupational Therapist.
- Proven experience in community-based occupational therapy.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Experience in managing complex caseloads is desirable.
