Job Overview

We are currently seeking a dedicated and skilled Band 6 Occupational Therapist for a community-based role at Leeds Community Healthcare. The successful candidate will work across various community settings, providing patient-centered care and promoting independence.

Key Responsibilities
  • Assess, plan, and deliver occupational therapy interventions for patients in the community.
  • Work with multi-disciplinary teams to support the rehabilitation and recovery of patients.
  • Develop and implement tailored care plans to meet individual needs.
  • Provide advice and education to patients, families, and carers.
  • Manage a caseload, ensuring timely and effective interventions.
Key Requirements
  • HCPC registration as an Occupational Therapist.
