Qualified Social Worker, Advanced Practitioner, Hospital TeamPay rate £35 per hour Contract role The Talent Crowd are recruiting for a Qualified Social Worker for a role in a Hospital Team in Leeds.We are looking for 3 social workers with experience of working in Adult Social Care, hospital discharge neighbourhood teams. The social worker should be experienced with good understanding of the social worker's processes including assessment, intervention and review and the key legislation that underpin this process. They should have the ability to carry a caseload of complex cases and undertake assessment of service users and carer needs using a person-centred approach in accordance with agreed statutory and directorate procedures.  Able to assess capacity and coordinate best interest meetings and decision making for those who lack capacity to make decision for their discharge destination. Able to assess and manage risks and ensure any safeguarding is dealt with in timely manner.
