- To take a key role in supporting the team alongside the team manager including supporting less experienced staff with complex cases.
- To support with allocations and attending partner meetings in the absence of the team manager.
- To be responsible for own case load which will be based both in hospital and outside of hospital. Such work will include supporting safe and timely discharges from hospital and reviewing care in temporary placements and in people's homes.
