We are seeking a Referral and Assessment Manager to lead the referral and assessment processes for individuals with Learning Disabilities (LD) and Autism.

You will be instrumental in ensuring smooth transitions for service users into their new homes and overseeing their movement within our services.

What You Will Do:• Manage Referrals & Assessments: Oversee all incoming referrals, ensuring a comprehensive and timely assessment process for potential service users.• Facilitate Transitions: Collaborate closely with families, healthcare professionals, and local authorities to ensure smooth and seamless transitions for service users.• Coordinate Placements: Ensure that service users' new homes meet their specific needs, managing placements and movement across our services.• Ensure Compliance: Maintain up-to-date records for all assessments, referrals, and placements while ensuring compliance with all regulatory and statutory requirements.• Foster Relationships: Build strong working relationships with commissioners, social workers, and other key stakeholders.• Support Operational Teams: Provide operational teams with guidance on person-centered care and tailored support plans.• Drive Service Improvement: Contribute to service development and improvement initiatives to continually enhance service quality.

What We Are Looking For:• Relevant Experience: Proven experience in managing referrals, assessments, and placements within Learning Disabilities and Autism services.• Knowledge of Best Practices: Strong understanding of safeguarding, regulatory requirements, and industry best practices.• Assessment Expertise: Skilled in assessing complex needs and creating suitable care plans.• Communication Skills: Excellent ability to build relationships with diverse stakeholders, including commissioners and multidisciplinary teams.• Organisational Skills: Highly organised with the ability to manage multiple referrals and placements efficiently.• Problem-Solving: Proactive in identifying and addressing challenges, particularly with service user transitions.• Travel Flexibility: Willingness to travel across multiple regions as needed.
