Job Summary:

A well-established and patient-focused medical practice in Leigh is looking for an experienced and dedicated Practice Nurse to join their healthcare team. The ideal candidate will play a key role in delivering exceptional care to a diverse patient community.

Key Responsibilities:

Deliver a wide range of nursing care services, including chronic disease management, wound care, immunizations, and health screenings.
Perform patient assessments, develop individualized care plans, and ensure the delivery of patient-centered care.
Work collaboratively with GPs, healthcare assistants, and other members of the multidisciplinary team to ensure seamless care delivery.
Provide patient education and health promotion, empowering patients to take an active role in their health and well-being.
Keep accurate and up-to-date patient records, ensuring adherence to all relevant guidelines and protocols.
Qualifications and Experience:

Registered Nurse (RGN) with a current NMC pin.
