Service Care Solutions is seeking a Memory Assessment Nurse for the Lancashire & South Cumbria NHS Foundation Trust located in Lancaster.
The position involves conducting dementia assessments as part of a multi-disciplinary team and requires a Mental Health Nursing degree along with 12 months of post-qualification experience.
The pay rate is £27 per hour with an additional £250 bonus for starting a contract.
Responsibilities include providing mentoring, identifying risk factors, and making appropriate referrals to other services as needed.
Candidates must have a fully enhanced valid DBS and experience working with older adults or dementia.

To have a specialist knowledge and understanding of strategies and interventions in the treatment of dementia.
To be proficient in referring to other agencies within the service or referring to other services or agencies where necessary.
To confidently identify risk factors and action accordingly.
To provide mentoring and clinical supervision to other staff and trainees on placement.

Qualifications and Requirements:

Fully enhanced valid DBS
Must have experience with Older adults / dementia
Must have Mental Health Nursing degree.
12 months post qualification experience required.
