Join our team as a Community Dietitian and make a real difference in the lives of patients requiring home enteral feeding. Your core responsibilities include:

• Conducting specialist nutritional assessments for patients requiring HEF.• Developing and implementing individualised feeding plans.• Providing education and support to patients, families, and carers.• Collaborating with multidisciplinary teams to optimise patient care.• Keeping accurate and up-to-date records for compliance and continuity of care.• Staying informed on the latest advancements in HEF and community dietetics.

Requirements & Qualifications:• Current and valid HCPC registration.• Experience in Home Enteral Feeding (HEF) and community dietetics.• Clinical NHS experience at Band 6 level.• Strong communication and interpersonal skills.• Ability to work both independently and as part of a multidisciplinary team.• Commitment to delivering high-quality patient care.
