About the Role

Are you passionate about making a real difference in people's lives? Our Client as a Community Care Officer Level 2, where you'll work closely with individuals, families, and communities to promote independence and well-being. This is an exciting opportunity to contribute to the Adult Care and Community Wellbeing Directorate, providing essential care and support services tailored to meet diverse needs.

Key Responsibilities:
  • Manage a caseload of service users with complex needs, identifying appropriate solutions to promote independence.
  • Conduct assessments of needs, including health, housing, and social care requirements.
  • Develop and implement tailored care packages in collaboration with service users, carers, and other agencies.
  • Provide advice, arrange equipment or minor adaptations, and teach techniques to enhance independent living.
  • Collaborate with multidisciplinary teams and represent the Directorate in multi-agency meetings.
  • Support qualified social workers and therapists, ensuring high standards of service delivery.
  • Maintain accurate records, monitor progress, and adapt care plans to achieve desired outcomes.
What We're Looking For:
  • A minimum of 4 GCSEs (Grade C or above) or equivalent, including English and Maths.
  • Experience in a strengths-based approach to care, promoting independence.
  • Knowledge of relevant health and social care legislation.
  • Effective IT skills and the ability to manage conflicting priorities.
  • Strong interpersonal and communication skills, with the ability to build positive relationships.
  • Understanding of confidentiality, safeguarding practices, and the rights of service users and carers.
