About the Role: We are seeking dedicated Advanced Clinical Practitioners (ACPs) to join telephone triage teams in Liverpool. In this vital role, you will be responsible for triaging patients entering the service from NHS 111, the Clinical Assessment Service, and various primary care settings. Your expertise will help minimize hospital admissions by directing patients to the most appropriate services.

Key Responsibilities:

  • Conduct telephone triage for patients, ensuring timely and effective care.
  • Prioritize patients according to service escalation protocols including elderly and palliative patients. 
  • Collaborate with multidisciplinary teams to streamline patient care pathways.
  • Provide clinical advice and support to patients and their families.

Essential Qualifications:

  • Minimum of 3 years post-qualification experience in emergency care and telephone triage.
  • Proven experience in urgent or primary care settings.
  • Ability to assess and treat children aged 2 years and older.
